Step 2 - Complete the below Acknowledgement Form

Conflict of Interest
A Conflict of Interest form is required to be submitted for all Pecuniary or Non-Pecuniary declarations of Interests (refer to Part 4 & 5 of the Code of Conduct).

Gift and Benefits Register
A Gift and Benefits form is required to be completed as per Part 6 of the Code of Conduct should you be the recipient of any gifts or benefits in your duty as a Committee member, as detailed in the Code of Conduct.
